ONLINE REGISTRATION FOR THE CDA 85th ANNUAL CONFERENCE  IS NOW CLOSED

All new registrations and change requests can now be made onsite at the conference registration desk at the Delta St. John’s Hotel.  Or you may fax in your registration form until Friday, June 25th to 604-685-3521.

Onsite Registration hours are:

Wednesday, June 30       2:00 PM - 7:00 PM
Thursday, July1              6:30 AM - 7:00 PM
Friday, July 2                  6:30 AM - 2:30 PM
Saturday, July 3              6:30 AM - 6:00 PM

Registration cancellation requests are 100% non-refundable.  Substitutions can be made with a $50 Administration fee.

Conference Registration Fees
Conference registration fees include all Scientific Meetings, Patient Clinical Session(with the exception of Medical Office Staff and Sponsors), Sub-Specialty Meetings and Symposia, Welcome Reception, Sponsor Hosted Evening and the President's Banquet.

Registration Confirmation Letters
Upon receipt of completed online or faxed/mailed forms and payment, a confirmation will be sent provided registration is received prior to or on June 25, 2010. Any registrations received after June 25, 2010 will be processed onsite.
